	Fills in rather soggy form Own Hair?......Checks mirror & adjusts toupee......Y.E.S Own Teeth?.....Still have most of them. Can't guarantee that in a couple of days though. Age.........45 crosses it out.....30 crosses it out they want fall for that.....35 yep, just say I've aged badly.
