I would say that the 'formerly' is her maiden name and she then married husband number 1. |

Formerly would be the maiden name, Late would be the surname possibly of a first husband. it’s the late(st) ie most recent name used. |

If a female is listed with a current married surname, late surname and formerly surname on a certificate which is the birth surname. Late or formerly. |
